Job summary

Transitions Hospice is seeking an adaptable ECT Nurse to join our team serving Ottawa, Streator, Dwight, Pontiac and surrounding areas. You will provide high-quality nursing across the full care process, collaborate with the interdisciplinary team, and respond quickly to patient needs.

You bring an active Illinois nursing license and hospice/home health experience, with strong assessment and documentation skills, plus a willingness to travel locally for varied cases.

• Hospice/home health experience preferred
• Flexible and able to work across varied assignments

Responsibilities

• Deliver professional nursing care across the full nursing process
• Conduct thorough patient and family assessments and contribute to care plans
• Complete all visit types as needed to meet patient and family needs
• Administer medications and treatments per physician orders
• Identify nursing diagnoses from health data and respond accordingly
• Document accurate clinical notes including pain and symptom outcomes
• Communicate condition changes to physicians and obtain orders as required
• Coordinate with community health partners for seamless transitions
• Ensure equipment and services are arranged for patients
• Maintain and grow clinical skills through continuing education

What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:

• Delivering professional nursing care across the full nursing process within your scope of practice
• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing all visit types as needed to meet patient and family needs
• Administering medications and treatments per physician orders
• Identifying nursing diagnoses from health assessment data and responding accordingly
• Recording accurate, timely clinical notes including pain and symptom management outcomes
• Communicating changes in patient condition to physicians and obtaining orders as required
• Coordinating with community health partners to support seamless care transitions
• Ensuring equipment and necessary services are arranged and available for patients
• Maintaining and growing your clinical skills through continuing education and in-service participation

What You Bring

• Active LPN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred
• Flexibility and confidence working across varied patient assignments
• Strong assessment skills and sound clinical judgment under pressure
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally
