Job summary

Transitions Hospice - Illinois is seeking an adaptable RN to join our Extended Care Team as an ECT Nurse in Illinois. You will provide critical nursing support across cases, assess patient needs with speed and clinical precision, and work within an interdisciplinary team to ensure comprehensive care.

You will deliver care across the full nursing process, communicate with physicians, document thoroughly, and coordinate with community partners to ensure smooth transitions.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.
• Flexibility and confidence across varied patient assignments.
• Strong assessment skills and clinical judgment under pressure.
• Excellent documentation habits and communication skills.
• Valid driver's license and willingness to travel locally.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and necessary services are arranged and available for patients.
• Maintain and grow clinical skills through continuing education and in-service participation.
