Vitality Hospice & Palliative Care is seeking a Registered Nurse Case Manager (RNCM) to join our compassionate team. This full-time position involves admissions, care planning, and collaboration with an interdisciplinary team to deliver high-quality hospice care in patients' homes.

The ideal candidate will hold an RN license and have hospice or home health experience, strong communication skills, and the ability to travel within the service area. Travel and scheduling flexibility are expected.

• Ability to travel within the assigned service area
