VitalCaring Group is seeking a dedicated Hospice RN to provide field-based, patient- and family-centered care in home and community settings. You will assess needs, tailor plans, and coordinate with physicians, social workers, chaplains, and aides to support end-of-life goals with compassion and clinical excellence.

This role offers autonomy within a collaborative interdisciplinary team, empowering you to make a meaningful difference for patients and families during life’s most important moments.

Qualifikationen

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach; ability to manage a caseload independently.
• Excellent communication skills in sensitive situations and Basic EMR proficiency.

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approachAbility to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation
