Job summary

Transitions Hospice - Illinois is seeking an RN Case Manager to serve Vandalia and surrounding areas. You will coordinate care, guide the interdisciplinary team, and respond when it matters most, delivering compassionate, patient-centered hospice services.

The role requires an active RN license in the state, plus corporate-aligned experience in hospice or home health; strong communication and clinical judgment are essential. Expect rotating schedule and competitive compensation.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• 1+ year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.

Salary Range: $70,000 - $85,000 a year
