Job summary

Transitions Hospice is seeking an RN Case Manager to serve Pontiac and surrounding areas. You will coordinate care, guide the interdisciplinary team, and advocate for patients and families in end-of-life care.

Requirements include active RN license in the state, graduation from an accredited nursing program, and 1+ year hospice/home health experience preferred, with strong communication and clinical judgment for independent field work.

• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conduct comprehensive, ongoing assessments of patients' evolving needs.
• Build and adjust personalized care plans with physicians and care teams.
• Administer medications and treatments per physician orders.
• Keep clinical documentation accurate, timely, and meaningful.
• Serve as the communication bridge between families, physicians, and community health partners.
• Educate patients and caregivers to feel confident and supported.
• Participate in on-call rotation per team policy.

Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $80,000 - $90,000 a year
