Transitions Care is a leading hospice provider, serving New Albany and surrounding areas with 24/7 home-based services. The RN Case Manager will plan, organize, and direct hospice care in patients' homes, collaborating with an interdisciplinary team and traveling locally to coordinate services for patients and families.

The role requires nursing graduation, active RN license in the state, and prior hospice or home health experience is preferred.

Qualifications

• Graduate of an accredited school of Nursing.
• Registered Nurse licensed in the State.

Responsibilities

• Completes ongoing comprehensive assessment of patient and family to determine hospice needs.
• Initiates the plan of care and revises as patient needs change.
• Administers medications and treatments as prescribed by the physician.
• Maintains and submits clinical notes on patient condition and care.
• Communicates with physician regarding patient needs and changes in condition.
• Coordinates care with community health professionals.
• Teaches patient and family/caregiver self-care techniques.
• Works with interdisciplinary team.
• Participates in on-call duties as defined by policy.
• Pursues ongoing professional development.

Schedule: M-F; 8 hour shifts (on call rotating)

Salary Range: $75,000 - $85,000 a year

As a Transitions Care RN Case Manager you will plan, organize, and direct hospice care. They work closely with the interdisciplinary team to plan and direct services to meet the needs of the patient and families within their homes and communities. Case Managers will travel locally and are responsible for the following:

Major Responsibilities

• Completes an ongoing comprehensive assessment of patient and family to determine hospice needs.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Administers medications and treatments as prescribed by the physician in the physician's plan of care.
• Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care given.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians' orders as required.
• Communicates with community health related persons to coordinate the care plan.
• Teaches the patient and family/caregiver self-care techniques as appropriate.
• Works in concert with the interdisciplinary group.
• Participates in on-call duties as defined by the on-call policy.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills obligation of requested and/or accepted case assignments.

Requirements

Education and Experience

• Graduate of an accredited school of Nursing.
• Registered Nurse licensed in the State.
• One (1) + year Hospice, home health or related organization experience preferred.
