Home Care Select Hospice is seeking a dedicated Registered Nurse to provide hospice care in patients' homes and community settings within the Greater Houston area. The RN will educate families, coordinate with an interdisciplinary team, and ensure regulatory compliance while delivering compassionate, patient-centered care in a home-based environment.

Qualifications

• RN license in Texas with hospice or palliative care experience.

• Strong organizational and documentation skills; able to manage multiple patients.

Responsibilities

Role Description

This full-time, on-site Registered Nurse role is based in the Greater Houston area and focuses on delivering hospice care in patients’ homes or designated care settings. The Registered Nurse will assess patient conditions, develop and implement individualized care plans, and provide skilled nursing interventions such as pain and symptom management. Daily responsibilities include administering medications, monitoring vital signs, documenting clinical findings, and coordinating with physicians, social workers, chaplains, and other team members to ensure integrated care. The nurse will educate patients and families on disease processes, treatment options, and comfort measures, while offering emotional support and guidance. The role also involves participating in care conferences, maintaining regulatory and safety standards, and contributing to quality improvement initiatives.

Qualifications

• Current, unrestricted Registered Nurse (RN) license in the state of Texas, with strong clinical assessment and hospice or palliative care skills.
• Experience providing bedside nursing care, including pain and symptom management, medication administration, and monitoring of complex conditions.
• Ability to communicate clearly and empathetically with patients, families, and interdisciplinary team members, both verbally and in writing.
• Strong organizational and time-management skills, with the ability to prioritize multiple patients and maintain accurate documentation.
• Comfort working in home and community settings, including reliable transportation and willingness to travel within Greater Houston.
• Knowledge of hospice regulations, infection control practices, and patient safety standards; familiarity with electronic health records is preferred.
• Demonstrated capacity for emotional resilience, cultural sensitivity, and respectful, non-discriminatory care for diverse populations.
• Basic Life Support (BLS) certification required; hospice or palliative care certification is an advantage.
• Associate or Bachelor’s degree in Nursing; relevant hospice experience strongly preferred.
