Key Responsibilities

• Align activities with Elara Caring’s vision and leadership
• Assess patient needs and develop plans of care
• Maintain patient and staff confidentiality per HIPAA
• Evaluate patients and adjust care plans accordingly
• Communicate significant findings and changes in health status
• Revise care plans and coordinate with team members

Minimum Requirements

• Current, unrestricted state RN license
• At least 1 year of clinical care experience as a nurse
• Willing to travel within the coverage area
• Dependable vehicle, valid driver’s license, and auto insurance
• Ability and willingness to travel 50%
• Associates or Bachelor’s Degree in Nursing (preferred)
• Home Health or Hospice experience (preferred)
• Experience with HomeCare HomeBase (preferred)

You will report to the Clinical Manager or Supervisor.
