Responsibilities:

• Align activities with Elara Caring’s vision and leadership directives
• Assess patients' physical, psychosocial, and environmental needs
• Manage a caseload including assessment, planning, implementation, and evaluation of care
• Evaluate patient needs and incorporate physical, psychological, social, and spiritual factors into the care plan
• Perform initial and follow-up evaluations, communicate significant findings
• Revise care plans as needed and notify relevant team members

Minimum Requirements:

• Current, unrestricted state RN license
• At least 1 year of clinical care experience as a nurse
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver's license, current auto insurance
• Willing to travel 50%
• Preferred: Associate’s or Bachelor’s Degree in Nursing
• Preferred: Home Health or Hospice experience, familiarity with HomeCare HomeBase

You will report to the Clinical Manager or Supervisor.
