The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with applicable laws, regulations, and policies. Responsibilities include:

• Coordinating the total plan of care with healthcare professionals
• Conducting initial and comprehensive nursing evaluations
• Developing and implementing the plan of care
• Performing patient assessments and ensuring eligibility and medical necessity

License Requirements:

• Current RN licensure in Oregon
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to reliable transportation
