Job summary

UPMC Home Health is seeking a full-time Registered Nurse to support Lower Burrell, New Kensington, and nearby communities. The role runs Monday–Friday, 8:00 AM–4:00 PM with on-call and weekend rotations, emphasizing one-on-one patient care in home settings.

You will collaborate with physicians and a full interdisciplinary team, develop care plans, educate patients and families, and document care electronically while meeting regulatory standards and pursuing growth within the organization.

Qualifications

• Active RN license for Pennsylvania or eligibility via compact state
• Graduate of an accredited school of professional nursing
• Valid driver’s license, auto insurance, and required clearances
• CPR certification per AHA standards
• Willingness to take on on-call and weekend rotations

Responsibilities

• Deliver skilled nursing care to patients in their homes across assigned Pittsburgh-area communities
• Develop and manage plans of care with physicians and interdisciplinary partners
• Provide patient and family education to promote independence and recovery
• Complete required documentation (OASIS, visit notes, orders) electronically using a provided laptop
• Maintain regulatory standards while focusing on high-quality, patient-centered outcomes
