Job summary

WSi Healthcare Personnel is seeking an experienced Registered Nurse (RN) to support a home health team in the Pueblo area. The primary need is for a full-time RN, with part-time options available based on staffing.

You'll complete Start of Care visits, recertifications, and perform comprehensive patient assessments in patients' homes, while ensuring accurate clinical documentation and coordination with the clinical team.

Qualifications

• Active RN license in Colorado.
• Home health experience preferred.
• Ability to work independently in patient homes.

Responsibilities

• Complete Start of Care (SOC) visits and recertifications.
• Perform patient assessments and provide skilled nursing care in the home.
• Review and complete required clinical documentation.
• Assist with chart review and ongoing care coordination.
• Communicate with the clinical team regarding patient needs and changes in condition.
• Maintain accurate and timely documentation.

This is a strong opportunity for an RN with home health experience who is comfortable working independently in the field and completing Start of Care visits, recertifications, and clinical documentation.
