Job summary

Evergreen At Home is seeking a compassionate Registered Nurse (RN) to provide skilled home health nursing across the community of Oshkosh, WI. You will coordinate with physicians and therapists to manage chronic conditions and enhance quality of life for patients in their own homes.

This full-time role offers competitive hourly pay, a daytime schedule, and a maximum driving radius of 45 minutes from Oshkosh, with paid vacation, sick time, and a 403(b) plan.

Qualifications

• Active Wisconsin Registered Nurse (RN) license.
• Current CPR certification.
• Valid Wisconsin driver’s license.
• Reliable transportation with appropriate auto insurance and a good driving record.
• Strong organizational and time-management skills.
• Passion for serving older adults and maintaining independence.

Responsibilities

• Provide skilled nursing care to older adults in patients' homes.
• Coordinate care plans with physicians, therapists, and other providers.
• Monitor patient conditions and communicate changes promptly.
• Educate patients and families on disease management, medications, and care plans.
• Complete accurate OASIS documentation.
• Assist with discharge planning and transitions of care.
• Connect patients with community resources and support services as needed.

Are you a compassionate Registered Nurse who enjoys building meaningful relationships with patients and helping them maintain independence in their own homes?

Evergreen At Home is currently seeking a Registered Nurse (RN) to provide skilled home health nursing services throughout the community. In this role, you'll work closely with physicians, therapists, patients, and families to coordinate care, manage chronic conditions, and improve quality of life.

What We Offer

• Competitive hourly pay: $37.50 - $42.50/hour, based on experience and number shifts
• Hourly compensation, including drive time and documentation time (not paid per visit)
• 30-40 hours per week, daytime schedule, you make your own daily schedule
• Small driving radius - no farther than 45 minutes from Oshkosh
• Weekly team meetings to discuss case load
