Evergreen At Home in Oshkosh, WI is seeking a full-time Home Health Nurse to provide skilled nursing services in clients' homes and coordinate care with a multidisciplinary team.

You will assess health status, develop and implement individualized care plans, administer medications, perform wound care, and educate patients and families on disease management and safety while documenting in electronic health records and participating in quality initiatives.

Qualifications

• RN license in Wisconsin, active and unrestricted.
• Experience in home health or related setting.
• Proficient in wound care and clinical assessments.
• Comfort using electronic health records and basic digital tools.
• Valid driver’s license and ability to travel to Oshkosh area.
• Strong communication and critical thinking, capable of independent work in a team.

Role Description

The Home Health Nurse is a full-time, hybrid role based in Oshkosh, WI, with a combination of in-home patient visits and care coordination responsibilities. Day-to-day tasks include assessing patient health status, developing and implementing individualized care plans, administering medications and treatments, and providing wound care and other skilled nursing services in the home setting. The nurse will educate patients and caregivers on disease management, medications, and safety, while collaborating with physicians, therapists, and other team members to ensure continuity of care. Additional responsibilities include accurate and timely charting in electronic health records, monitoring patient progress, responding to changes in condition, and participating in case conferences and quality initiatives.

Qualifications

• Strong Nursing and Medicine skills, with the ability to perform comprehensive assessments and deliver evidence-based clinical care
• Experience in Home Care and Patient Care, including supporting individuals in their residences and promoting independence and safety
• Proficiency in Wound Care, including assessment, dressing changes, infection prevention, and monitoring healing
• Current Registered Nurse (RN) license in the state of Wisconsin and eligibility to practice without restriction
• Prior home health, acute care, or long-term care experience preferred
• Comfort with electronic health records and basic digital tools for documentation and communication
• Valid driver’s license, reliable transportation, and ability to travel within the Oshkosh, WI area
• Strong communication, time management, and critical thinking skills, with the ability to work independently and as part of a multidisciplinary team
