Transition Home Healthcare in Houston, TX is seeking a dedicated Registered Nurse (RN) for home health services. You will oversee patient care planning, coordinate with physicians, and lead the nursing care team in the patient’s home.

Requirements include an active Texas RN license, at least 1 year of home health experience, knowledge of home health regulations, and proficiency with HCHB EMR. We offer mileage reimbursement, paid time off, health benefits, and a 401(k) with match.

Responsibilities:

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Qualifications:

• Valid RN license in the State of Texas or compact license.
• Home health experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR (preferred).
• Excellent communication and people skills are essential.
• Ability to work independently and as part of a team.
