Transition Home Healthcare in Houston, TX is hiring a Registered Nurse (RN) - Home Health. This full-time role offers PRN or full-time shifts and serves the Inner Loop area, delivering high-quality in-home patient care and coordinating with the care team.

Responsibilities include leading the nursing care team, developing care plans, educating patients and families, and ensuring complete clinical notes. Requires Texas RN license (or compact), at least 1 year of home health experience, and strong

Qualifications

• Valid RN license in Texas (or compact license).
• Minimum 1 year home health experience.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR preferred.
• Excellent communication and people skills; able to work independently and in a team.

• Oversee nursing care team.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
