Transition Home Healthcare in Houston, TX is seeking a dedicated Registered Nurse (RN) to oversee home health services in the Inner Loop area. This full-time role offers growth opportunities, collaboration with physicians and care teams, and a commitment to high-quality patient care.

Responsibilities include directing nursing care, planning and evaluating patient care plans, educating families, and ensuring accurate clinical documentation.

Qualifications

• Valid RN license in Texas or a compact license.
• Home health experience for 1+ year.
• Knowledge of home health regulations and general healthcare compliance.
• Experience with HCHB EMR is preferred.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists, and other healthcare professionals.
• Plan, organize, and direct home care services.
• Assess patient care needs and develop care plans.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.

Qualifications
