Job summary

Transition Home Healthcare in Houston, TX is seeking a licensed LVN to join our elite home health team. We offer PRN and Full-Time shifts serving the North, Northwest, Northeast, Southwest, and Inner Loop areas.

You will provide patient-centered care, conduct visits, and collaborate with the care team to implement individualized plans. The role emphasizes compassionate communication with patients and families, thorough documentation, and adherence to home health regulations.

Qualifications

• Valid and current LVN license in TX or compact state.
• Minimum of 1 year of home health experience.
• Knowledge of home health regulations and healthcare compliance.
• Experience with HCHB EMR (preferred).
• Excellent communication and people skills with patients and families.
• Compassionate, empathetic demeanor focused on patient-centered care.

Responsibilities

LVN (Home Health)

Shifts Available: PRN & Full Time

Responsibilities:

• Collaborate with the home healthcare teamto develop and implement individualized care plans.
• Complete home care nursing visits.
