All About Home Care is a Houston-based home health care agency seeking Registered Nurses to serve patients in the Northwest area of Greater Houston. We value candidates with home health experience and require a Texas RN license. Kinnser software is used to document patient care.

Responsibilities include initial evaluations, ongoing assessments, implementing skilled nursing care, and coordinating with therapists and physicians. Flexible scheduling is offered to support caregivers and patients.

Formación

• Graduate of an approved school of professional nursing and currently licensed in the state of Texas.
• Home Health skilled nursing experience required, RN.
• High clinical skills with emphasis on wound care/VAC & IV.
• RNs must have OASIS experience
• Ability to exercise initiative and independent judgment.

Responsabilidades

• Provide services per plan of care.
• Initial evaluation visit and regular reevaluations.
• Deliver those services requiring skilled nursing skills.
• Initiate preventive and rehabilitative nursing skills.
• Prepare clinical and progress notes and summaries of care conferences.
• Inform personnel of changes in patient conditions.
• Counsel the patient and family in nursing needs.
• Participate in in-service programs.
• Adhere to agency policies and procedures.
• Process orders and notify physician of patient needs and changes.
• Complete certification/recertification orders and discharge summaries.
• Determine nursing needs for each patient.
• Refer to therapists for specialized skills.
• Supervise and teach other nursing personnel.
• Conduct patient care conferences and participate in QA as assigned.
• Gives total patient care as needed.

Summary:

All About Home Care is a Houston-based home health care agency. We've been serving the Houston and surrounding communities since 2004, providing quality home health services. We are currently looking for a Registered Nurses (RNs) to help serve patients in the Northwestarea of Greater Houston. If you have experience in home health, please submit your resume. We use Kinnser software to document patient care.

Qualifications:

Responsibilities:

• Provide services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient's nursing needs.
• Provides those services requiring substantial specialized nursing skills.
• Initiates appropriate preventative and rehabilitative nursing skills.
• Prepares clinical and progress notes for each patient's visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
• Informs personnel in changes in the conditions and needs of the patient.
• Counsels the patient and the family/significant others in meeting nursing and related needs.
• Participates in and presents in-service programs.
• Understands and adheres to established Agency policies and procedures.
• Process orders and notifies physician of patient needs and changes in condition.
• Complete certification/re-certification orders and discharge summaries.
• Determines the amount and type of nursing needed by each individual patient.
• Refers to Physical Therapist, Speech Language Pathologist, Occupational Therapist, and Medical Social Worker those patients requiring their specialized skills
• Supervises and teaches other nursing personnel.
• Conducts patient care conferences on patients assigned to his/ her care.Participates in peer review and performance improvement as assigned.
• Gives total patient care as needed.
