Job summary

All About Home Care Inc. in Houston is seeking an experienced RN to cover the Upper Central North Greater Houston area, including Spring, Conroe, and The Woodlands.

You will evaluate patients, implement the plan of care, and coordinate with therapists to deliver skilled nursing services in patients’ homes. You must be Texas-licensed with home health experience and strong wound care skills, and be able to perform OASIS assessments with independence.

Qualifications

• Graduate of an approved school of professional nursing and currently licensed in the state of Texas.
• Home Health skilled nursing experience required, RN.
• High clinical skills with emphasis on wound care/vac & IV.
• RNs must have OASIS experience
• Ability to exercise initiative and independent judgment.

Responsibilities

• Provide services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the patient’s nursing needs.
• Provides those services requiring substantial specialized nursing skills.
• Initiates appropriate preventative and rehabilitative nursing skills.
• Prepares clinical and progress notes for each patient’s visit and summaries of care conferences on his/her patients in a timely manner as per Agency policy.
• Informs personnel in changes in the conditions and needs of the patient.
• Counsels the patient and the family/significant others in meeting nursing and related needs.
• Participates in and presents in-service programs.
• Understands and adheres to established Agency policies and procedures.
• Process orders and notifies physician of patient needs and changes in condition.
• Complete certification/re-certification orders and discharge summaries.

is a Houston-based home health care agency. We have been serving Houston and the surrounding communities since 2004, providing quality home health services. We are looking for Registered Nurses (RNs) to help care for patients in the Upper Central North of Greater Houston, which mainly includes Spring, Conroe, The Woodlands, and surrounding areas. If you are comfortable covering the said areas and have experience in home health. We use Kinnser software to document patient care.

• Conducts patient care conferences on patients assigned to his/ her care.
• Participates in peer review and performance improvement as assigned.
• Gives total patient care as needed.
