Job summary

Empath Home Health in Fort Myers, FL, is seeking a compassionate Home Health RN to deliver individualized in-home care across Lee County. The role focuses on comprehensive assessments, plan-of-care development, and patient/family education to support recovery and independence.

You will collaborate with an interdisciplinary team to provide holistic, Full Life Care within a mission-driven organization, including on-call rotations weekends and holidays.

Qualifications

• Active Florida RN license and graduation from an approved nursing program.
• Minimum 1 year of direct patient care; home health preferred.
• Willingness to participate in weekend/holiday on‑call rotations.

Responsibilities

Empath Home Health, a member of Empath Health is seeking a compassionate Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in Lee county (Fort Myers area).

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits throughout Lee county (including Fort Myers, Lehigh Acres, LaBelle, Cape Coral, Bonita Springs, Estero, Sanibel, and surrounding areas).

Schedule: M-F 40 hours with weekend/holiday on-call rotations required.

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self‑care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You’ll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on‑call rotations
