Job summary

WSi Healthcare Personnel is seeking an experienced Registered Nurse (RN) to support a home health team in the Pueblo area. The primary need is for a full-time RN, with part-time consideration based on availability.

You will perform Start of Care visits, recertifications, and complete clinical documentation in the field. The ideal candidate works independently, communicates clearly with the clinical team, and maintains accurate records.

Qualifications

• Current RN license in good standing.
• Experience in home health preferred.
• Ability to work independently in patient homes.

Responsibilities

• Complete SOC visits and recertifications in patients' homes.
• Perform patient assessments and provide skilled nursing care.
• Review and finalize required clinical documentation.
• Assist with chart review and ongoing care coordination.
• Communicate patient needs and condition changes to the clinical team.
• Maintain accurate and timely documentation.
