Elara Caring is seeking a Registered Nurse (RN) Home Health in Michigan for full-time work delivering at-home nursing care. You will assess, plan, implement, and monitor patient care while coordinating with physicians and families.

You will educate patients and families, maintain HIPAA compliance, and travel to patient homes using reliable transportation. A valid RN license and nursing degree are required; home health experience is a plus.

Qualifications

• Current State license as an RN.
• Graduate of an accredited nursing program (certificate/diploma/associate/baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home health experience preferred but not required.
• Ability to lift up to 50 pounds and travel between patient homes.

As a Registered Nurse Home Health ,you'llcontribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
