Job summary

Elara Caring is seeking a Registered Nurse Home Health to deliver high-quality care in patient homes. You will assess needs, implement plans, monitor conditions, and educate patients and families, while maintaining HIPAA privacy and collaborating with the care team.

Qualifications include current RN licensure, 1+ year in clinical care, and ability to lift up to 50 pounds; Home Health experience preferred but not required.

Qualifications

• Graduate of an accredited nursing program (Certificate/ Diploma/ Associate/ Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Home Health experience preferred but not required.
• Ability to lift up to 50 pounds and travel between patient homes.

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.

• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
