Job summary

Elara Caring is seeking a Registered Nurse Home Health to join our team delivering in-home nursing care across Michigan. You will assess patients, perform initial and ongoing evaluations, and provide care per physician orders while protecting privacy and documenting changes for the care team.

You will educate patients and families, respond to changing conditions, and strive to maintain high-quality, safe home health services on a travel-based schedule.

Qualifications

• Current State license as a Registered Nurse.
• 1+ year of nursing experience in a clinical setting.
• Home Health experience preferred but not required.

Responsibilities

• Assess patients to determine care needs and plan.
• Complete initial and ongoing patient assessments.
• Protect patient privacy per HIPAA.
• Provide nursing care per plan of care and orders.
• Monitor patient conditions and report changes to care team.

As a Registered Nurse Home Health , you’ll contribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.

• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
