Job summary

Elara Caring in Texas is seeking a Registered Nurse Home Health to provide high-quality nursing care in patients’ homes. You will assess, plan, implement, and evaluate care according to physician orders and the patient plan of care, while ensuring HIPAA privacy and safety.

This role requires current RN license, a nursing degree, at least 1 year of clinical experience, and willingness to travel between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Nursing program (Certificate, Diploma, ADN or BSN).
• Minimum 1 year of nursing experience in a clinical setting.
• Home Health experience preferred but not required.

Responsibilities

• Assess patients and determine care needs.
• Complete initial and ongoing patient assessments.
• Protect patient privacy under HIPAA.
• Provide nursing care per plan of care and physician orders.
• Monitor patient conditions and report changes to the care team.
• Educate patients and families on care and safety.

As a Registered Nurse Home Health ,you'llcontribute to our success in the following ways:

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
