• Teach, support, and guide patients on medications and home care.
• Communicate with providers, families and teams to coordinate care.
• Document visits accurately and reflect on outcomes for continuity.

What We're Looking For

• Active Registered Nurse (RN) license in Massachusetts.
• One year of nursing experience (home health or psych preferred).
• A drive to provide compassionate, patient‑centered care.
• Reliable transportation for patient visits.
