Qualifications

• Active Registered Nurse (RN) license in Massachusetts (MA).
• Minimum of 1 year of nursing experience (home health or psych preferred).
• Demonstrates compassionate, patient-centered care and reliable transportation.

Responsibilities

• Assess and adapt during visits, evaluating current condition and needs.
• Create and modify care plans based on patient needs and changes in condition.
• Teach and support patients with medications, wound care, and self-management.
• Communicate and coordinate with providers, families, and care teams to ensure appropriate care.
• Document and reflect on visits, submitting notes accurately for continuity of care.
