Comfort Home Care in the Somerville/Medford area is seeking a full-time Registered Nurse (RN) for an afternoon/evening weekday shift. This role offers a competitive base salary with an uncapped incentive plan and the opportunity to impact patients in their homes.

Qualifications

• Active Registered Nurse (RN) license in Massachusetts.
• One year of nursing experience (home health or psych preferred).
• Reliable transportation for patient visits.

Responsibilities

• Assess and Adapt: evaluate patient condition, medical needs, and well-being at each visit.
• Create and Modify Care Plans: actively shape treatment plans as conditions change.
• Teach and Support: provide medication guidance and patient education to promote independence.
• Communicate and Coordinate: inform providers, families, and care teams to ensure appropriate care.
• Document and Reflect: complete visit notes from home for compliance and continuity.
