Job summary

SummitWest Care in Mesa County, CO is seeking a Registered Nurse (RN) to provide skilled, patient-centered home health care across the county. You’ll deliver one-on-one care, develop personalized plans, and collaborate with an interdisciplinary team to ensure quality outcomes.

You’ll have flexible day-time scheduling within a Monday–Friday framework, on-call rotation, and mileage reimbursement as part of our supportive, nonprofit environment that values clinical autonomy and compassionate care.

Qualifications

• Active Registered Nurse license in Colorado or active NLC Compact license.
• Valid driver's license, reliable transportation, and auto insurance.
• Strong assessment and critical thinking skills.
• Ability to work independently with a structured caseload and clear communication.

Responsibilities

• Perform comprehensive patient assessments.
• Develop and update individualized plans of care.
• Provide skilled nursing care and appropriate interventions.
• Monitor patient progress and communicate changes to the care team.
• Educate patients and families regarding medications, treatments, and care plans.
• Coordinate care with LPNs, therapists, caregivers, and providers.
• Complete timely and accurate EMR documentation.
• Participate in on-call rotation.

$35–$43/hour (DOE) + Mileage Reimbursement

SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care throughout Mesa County. Home health offers a different nursing experience. Instead of rushing between rooms or managing overwhelming patient assignments, you'll provide meaningful one-on-one care, build lasting relationships with patients and families, and see the impact of your work firsthand. What We're Looking For

• Active Registered Nurse (RN) License in the State of Colorado (or active NLC Compact License)

• Home health experience a bonus, but not required
