Job summary

SummitWest Care, a nonprofit home health agency in Grand Junction, CO, is seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This in-person home health role offers a flexible Monday–Friday day schedule with on-call rotation and a manageable caseload for meaningful one-on-one care.

Responsibilities include comprehensive patient assessments, care planning, skilled nursing interventions, and education for patients and families.

Qualifications

• Active RN license in CO or active NLC compact license.
• Valid driver's license with reliable transportation and auto insurance.
• Strong assessment and critical thinking abilities.
• Ability to work independently with a structured caseload.
• Clear, professional communication with patients and team members.
• Comfort with documentation and EMR systems (HCHB preferred).
• Home health experience preferred but not required.
