Job summary

SummitWest Care in Mesa County, CO is seeking a Registered Nurse (RN) to provide skilled, patient-centered home health care, full-time or part-time, with a Monday–Friday day schedule and on-call rotation. You’ll enjoy one-on-one patient care, clinical autonomy, and the support of an interdisciplinary team.

You’ll earn $35–$43 per hour DOE, plus mileage reimbursement, with a flexible schedule and a focus on quality care.

Qualifications

• Active RN license in Colorado (or active NLC Compact)
• Valid driver's license and reliable transportation
• Strong assessment and critical thinking skills
• Ability to work independently and manage a structured caseload
• Clear, professional communication with patients, families, and team members
• Documentation and EMR experience (HCHB preferred)
• Home health experience a bonus but not required

Responsibilities

• Perform comprehensive patient assessments
• Develop and update individualized plans of care
• Provide skilled nursing care and appropriate interventions
• Monitor patient progress and communicate changes to the care team
• Educate patients and families regarding medications, treatments, and care plans
• Coordinate care with LPNs, therapists, caregivers, and providers
• Complete timely EMR documentation and participate in on-call rotation

What We're Looking For
