Job summary

Springdale Health and Rehabilitation Center is seeking a skilled Registered Nurse (RN) to provide direct nursing care to residents in our long-term care and rehabilitation facility. You will assist with rehabilitation plans and collaborate with the clinical team to ensure high standards of care.

The RN will deliver patient-centered care, assess and document health conditions, administer medications, and work with physicians and therapists to implement individualized care plans.

Qualifications

• Valid RN license required.
• Strong clinical skills in geriatric and rehabilitation care.
• Excellent communication and interpersonal skills.
• Ability to work collaboratively in a team.
• Compassionate, patient-focused approach to care.
• Current BLS/CPR certification required.

Responsibilities

• Deliver patient-centered nursing care to residents in a long-term care and rehabilitation setting.
• Assess, monitor, and document residents' health conditions and progress.
• Administer medications and treatments as prescribed, ensuring compliance with facility protocols.
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Provide education and support to residents and their families regarding care plans and health conditions.
• Maintain accurate and timely documentation in compliance with state and federal regulations.
• Foster a safe, comfortable, and therapeutic environment for residents.
• Supervise and mentor nursing assistants to ensure quality care delivery.
