Crossroads Hospice is seeking a Registered Nurse Case Manager to provide direct patient care in homes or care facilities. You will listen attentively to patients' needs, respect their wishes, and serve as the primary communication link within our interdisciplinary team of holistic care providers.

Qualifications

• Six months minimum prior skilled nursing experience.
• Active, unencumbered state Registered Nurse license.
• Current driver's license with auto liability coverage.

Registered Nurse Case Manager Qualifications:

• Six (6) months minimum prior skilled nursing experience
• An active, unencumbered state Registered Nurse license

• Interest and Ability in providing patient care in homes and facilities

Registered Nurse Case Manager Schedule & Benefits:

• Monday to Friday, 8:30 AM to 5:00 PM
• No Mandatory on-call required
