Job summary

Crossroads Hospice & Palliative Care is seeking a Registered Nurse Case Manager to deliver direct patient care in patients' homes or care facilities. You will build trusting relationships with patients and families, and coordinate care as the lead communicator within a holistic team.

The role emphasizes compassionate end-of-life care, respect for patient wishes, and collaboration with an interdisciplinary group to ensure meaningful support beyond medical needs.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and Ability in providing patient care in homes and facilities.

Registered Nurse Case ManagerQualifications:

Registered Nurse Case Manager Schedule & Benefits:
