Job summary

Crossroads Hospice seeks Registered Nurse Case Managers to oversee in-home patient care and collaborate with an interdisciplinary team. The role emphasizes listening to patients and families, building trust, and coordinating care to honor wishes with dignity.

Schedule is Monday to Friday, 8:30 AM - 5:00 PM, with benefits including health, dental, vision, 401k, PTO, CE reimbursement, and opportunities for growth within the organization.

Qualifications

• 6 months minimum prior skilled nursing experience.
• An active unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.

Responsibilities

• Coordinate in-home patient care with families and care teams.
• Serve as central point of communication within the interdisciplinary team.
• Provide compassionate hospice nursing focused on dignity and comfort.

Registered Nurse Case Managers provide more than just medical assistance; they also listen attentively to patients' needs and honor their wishes. Registered Nurse Case Manager Qualifications:
