Job summary

Crossroads Hospice & Palliative Care in Ohio is seeking a Registered Nurse Case Manager to deliver direct patient care in homes or care facilities, prioritizing comfort and respecting patient wishes.

You will build trusting relationships with patients and families, serving as the primary communication link within our interdisciplinary holistic care team to support meaningful, patient-centered end-of-life goals.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• Active, unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and ability in providing patient care in homes and facilities.

Responsibilities

• Provide direct patient care in patients' homes or care facilities.
• Listen attentively to patients' needs and respect their wishes.
• Serve as the primary communication link within the interdisciplinary care team.

Registered Nurse Case Manager Qualifications

Registered Nurse Case Manager Schedule & Benefits
