Job summary

Crossroads Hospice seeks a Registered Nurse Case Manager to deliver direct patient care in homes and care facilities around Cleveland. You will collaborate with an interdisciplinary team, listen to patient needs, and honor their wishes while providing compassionate end-of-life care.

The role offers a Monday–Friday schedule, 8:30 AM–5:00 PM, with no mandatory on-call, plus a benefits package including health, dental, vision, 401k and PTO.

Qualifications

• Six (6) months minimum prior skilled nursing experience.
• An active, unencumbered state Registered Nurse license.
• Current driver's license and auto liability coverage.
• Interest and Ability in providing patient care in homes and facilities.

As a Registered Nurse Case Manager, you will provide direct care to patients in their homes or care facilities. Your role goes beyond offering medical assistance; it involves attentively listening to patients' needs and respecting their wishes. Building trusting relationships with patients and their families is a cornerstone of this position, allowing you to serve as the primary communication link within our interdisciplinary team of holistic care providers.

Registered Nurse Case Manager Qualifications:
