Job summary

The role requires an active Arizona (or Compact) RN license, ADN, and BLS, with ACM preferred. Two years of RN and CM experience are required, and Bachelor’s or Master’s degree is preferred.

Qualifications

• Current Arizona or Compact State RN license with Arizona endorsement.
• Bachelor's or Master's degree preferred for advancement.
• BLS within 30 days of hire required; ACM within 2 years of hire preferred.
• 2 years of RN experience and Case Management experience required.

Responsibilities

• Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors.
• Leads and coordinates discharge planning, including patient transfers and referrals to community resources; provides discharge education.
• Performs chart reviews to identify issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
• Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities, plans, and outcomes.

The RN Case Manager is a hybrid position, supporting patients and family members in both the inpatient setting and the emergency department. In this role, you will enjoy a collaboration amongst a multidisciplinary team to evaluate appropriateness of medical care, including admission, length of stay, transfer and discharge. Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. ***Per Diem Position***

What we're looking for

• Applicants should have a current Arizona or Compact State RN license with Arizona endorsement and possess an Associate's Degree in Nursing (ADN) from an accredited nursing school.
• Education: Bachelor's or Master's Degree preferred.
• Certifications: Basic Life Support (BLS) within 30 days of hire required and Accredited Case Manager (ACM) within 2 years of hire preferred.
• Experience: 2 years of RN experience and Case Management experience required.
