Job summary

Havasu Regional Medical Center is seeking a Registered Nurse Case Manager for a hybrid role serving both inpatient and emergency department patients in Lake Havasu City, AZ. The role requires an active Arizona (or Compact) RN license, ADN, and BLS, with ACM preferred. Two years of RN and CM experience are required, and Bachelor’s or Master’s degree is preferred.

Qualifications

• Current Arizona or Compact State RN license with Arizona endorsement.
• Bachelor's or Master's degree preferred for advancement.
• BLS within 30 days of hire required; ACM within 2 years of hire preferred.
• 2 years of RN experience and Case Management experience required.

Responsibilities

• Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors.
• Leads and coordinates discharge planning, including patient transfers and referrals to community resources; provides discharge education.
• Performs chart reviews to identify issues related to service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.
• Communicates effectively with providers, payors, and ancillary services to remove barriers and ensure timely care progression.
• Accurately documents case management activities, plans, and outcomes.
