Job summary

Havasu Regional Medical Center in Lake Havasu City, AZ is seeking a Registered Nurse Case Manager (RN) PRN to join our team. This hybrid role supports patients in inpatient and ED settings, focusing on appropriate care, discharge planning, and coordination.

You will collaborate with a multidisciplinary team, document activities, and ensure smooth transfers; a current RN license in AZ or compact state is required, with ADN or higher, BLS, and CM experience preferred.

Qualifications

• Current Arizona or compact state RN license with AZ endorsement.
• ADN from an accredited nursing school.
• BLS certification within 30 days of hire.
• ACM certification within 2 years of hire preferred.
• 2 years RN and case management experience.

Responsibilities

• Leads discharge planning and coordinates patient transfers.
• Communicates with patients, families, payors; addresses questions/concerns.
• Performs chart reviews to monitor service delivery and outcomes.
• Documents case management activities, plans and outcomes.
• Collaborates with providers and ancillary services to progress care.
