Job summary

Pathways Hospice seeks an experienced RN Case Manager to lead nursing care teams and coordinate care across Northwest, Southwest and Southeast regions. This full-time role emphasizes clinical leadership and collaboration with physicians, therapists and other health professionals.

You will assess patient needs, develop care plans, educate families, and ensure timely clinical notes. Weekend/night coverage and on-call rotation are required; TX RN license and hospice experience preferred;

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR.
• Excellent communication and people skills; ability to work independently and in a team.

Responsibilities

• Oversee nursing care team.
• Collaborate with physicians, therapists and other professionals for cohesive patient care.
• Plan, organize and direct home care services.
• Assess patient care needs and develop care plans.
• Educate patients and families on health management and medications.
• Prepare timely, complete clinical notes.
• Night and weekend shifts required; on-call rotation weekly.

Hospice - RN Case Manager

Responsibilities:

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing their health conditions, medication regimens, and any necessary lifestyle changes.
• Prepare timely, complete, and accurate clinical notes.
• Night and Weekends are required per hospice needs of patients
• If working the weekend-will receive the following Monday off (Admissions Included)
• Must be willing to participate in an On Call Rotation (Weekly Rotation)
