Job summary

Pathways Hospice is seeking an experienced RN Case Manager to oversee nursing care and coordinate comprehensive home health services for patients across a defined territory. You will collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive patient care and develop individualized care plans.

The role requires an active Texas RN license and hospice experience, with knowledge of home health regulations and HCHB EMR.

Qualifications

• Valid RN license in Texas.
• Hospice experience (at least 1 year).
• Knowledge of home health regulations and general healthcare compliance.
• Experience with Home Care Home Base (HCHB) EMR.
• Excellent communication and people skills.
• Ability to work independently and as part of a team.

Responsibilities

• Oversee nursing care team.
• Communicate and collaborate with physicians, therapists, and other healthcare professionals to ensure cohesive and comprehensive patient care.
• Plan, organize, and direct home care services.
• Conduct assessments of patient care needs and develop care plans.
• Educate patients and their families on managing health conditions and medications.
• Prepare timely, complete, and accurate clinical notes.
• Night and weekends are required per hospice needs of patients.
• If working the weekend, the following Monday off (Admissions included).
• Must be willing to participate in an on-call rotation (weekly).
