Job summary

St. Croix Hospice seeks a compassionate Registered Nurse Case Manager to deliver holistic in-home hospice care, partnering with a care team to create personalized plans and lead end-of-life conversations with empathy and clinical excellence.

You will manage assessments, care plans, medication administration, and caregiver education, while rotating on-call duties to ensure seamless transitions for patients and families in Missouri.

Qualifications

• RN licensure required in state of practice.
• 1+ years of acute care experience; hospice/home health experience preferred.
• Reliable transportation, valid driver’s license, and proof of auto insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family and update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
