Job summary

UP Health System - Marquette is seeking an experienced Registered Nurse (RN) Case Manager to join our Acute Care team in Marquette, Michigan. This full-time role focuses on patient flow, discharge planning, and care coordination to ensure timely discharges and optimal outcomes.

You will collaborate with physicians and care teams, monitor patient status, and participate in education and resource referrals. Michigan RN license and BLS-HCP are required; five years of hospital experience is

Qualifications

• Current Michigan RN license
• Associate's degree from an accredited nursing school (Bachelor's preferred)
• BLS Healthcare Provider (BLS-HCP) certification
• Five years nursing experience (hospital preferred)

Responsibilities

• Monitors and manages patient care to promote continuity of care, optimal outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints from patients, families, and payors regarding care.
• Participates in discharge planning and coordinates patient transfers and community resources; provides discharge education and referrals.
• Performs chart reviews to identify issues with service delivery, outcomes, satisfaction, and reimbursement.

Education

Associate degree in nursing

How You'll Contribute

A RN Case Manager who excels in this role:

What We're Looking For

Applicants should have a current Michigan RN license and possess an associate's degree from an accredited nursing school (bachelor's preferred).

Additional Requirements Include

• Basic Life Support Health Care Provider (BLS-HCP)
• Five (5) years nursing experience (hospital preferred)
