Job summary

UP Health System - Marquette is seeking an experienced Registered Nurse Case Manager to support patient flow and outcomes. You will conduct clinical assessment, collaborate with physicians and care teams, and manage utilization to ensure appropriate care and timely discharges.

The role emphasizes discharge planning, patient and family education, and coordination with community resources to optimize transitions and minimize delays.

Qualifications

• Current Michigan RN license.
• Associate's degree in nursing (Bachelor's degree preferred).
• BLS-HCP certification.
• Five years nursing experience (hospital setting preferred).

Responsibilities

• Monitor and manage patient care to promote continuity of care, optimal outcomes, satisfaction, cost efficiency and compliance.
• Consult with nursing staff and multidisciplinary teams to evaluate patient status and appropriateness of medical care.
• Monitor patient and family satisfaction and respond to questions from patients, families, and payors.
• Participate in discharge planning including patient transfers and coordinating community resources.
• Perform chart reviews to identify issues with service delivery, outcomes, satisfaction, cost and reimbursement.

Job Type: Full-Time 1.0 FTE or 80 hours/pp M-F

Schedule: 10 x 8-hour Day shifts or 8 x 10-hour Day Shifts/pp, Flexible Start Times, Every 4th Weekend and two (2) Holidays

Our Acute Care RN Case Managers are essential partners in patient flow and clinical outcomes. Using expert clinical assessment, real time collaboration with physicians and care teams, and strong utilization management skills, they ensure patients receive the right level of care while supporting timely discharges, reducing avoidable delays, and improving the overall hospital experience.

How You'll Contribute

A RN Case Manager who excels in this role:

• Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.
• Consults with nursing staff and multidisciplinary team regularly to evaluate patient's status and appropriateness of medical care, including admission, length of stay, transfer and discharge.
• Monitors patient and family satisfaction. Responds to questions and complaints from patients, family members, and payors regarding care.
• Participates in discharge planning including coordinating patient transfers to other facilities and coordinating community resources. Provides discharge education and resource referrals to patients.
• Performs chart review to identify actual or potential issues with service delivery, patient outcomes and satisfaction, compliance, cost, and reimbursement.

What We're Looking For

Applicants should have a current Michigan RN license and possess an associate's degree from an accredited nursing school (bachelor's preferred).

Additional Requirements Include

• Basic Life Support Health Care Provider (BLS-HCP)
• Five (5) years nursing experience (hospital preferred)
