Adaptive Home Health, LLC is seeking a Registered Nurse (RN) - Case Manager for a full-time position based in Detroit. The RN will focus on delivering excellent home health care, managing caseloads, and supervising LPNs. A significant part of the role includes conducting in-home assessments while ensuring high patient care standards.

The position offers an approximate yearly pay of $85,000-$105,000, with additional mileage compensation and bonuses. Candidates should have an active Michigan RN license and home health experience.

• Role: Registered Nurse (RN) - Case Manager

• Comp: $85,000-$105,000 approximate yearly pay

  • $64.10 per point

• Mileage in addition to productivity comp

• Schedule: Full-time Monday–Friday + some weekends (as needed)

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.

2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).

• Direct, high-acuity patient care, with a focus on wound care

• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.

• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.

Requirements

• Active Michigan RN license

• Reliable transportation, valid driver's license, auto insurance

• Home health / OASIS experience strongly preferred

Our guarantee

• Full-time target: ~30 productivity points/week; clinicians earn more for exceeding it.

• Guaranteed floor: ~21 points/week for the first 90 days - if your caseload hasn't filled, you're still paid the floor.
