Adaptive Home Health is expanding in Detroit and seeks a Field-based Registered Nurse (RN) Case Manager to deliver in-home care with our AI-native platform. You will conduct assessments, manage high-acuity cases, and collaborate with physicians to build plans of care while supervising LPNs and HHAs.

The role focuses on reducing charting time through ambient AI documentation and after-visit notes, enabling you to spend more time with patients and improving outcomes.

Qualifications

• Active Michigan RN license required.
• Experience in home health or OASIS strongly preferred.
• Ability to manage full caseload and supervise LPNs/HHAs.

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality, with OASIS accuracy that holds up to review.

2. Own your patients\' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.

3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.

4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you\'ll own

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).

• Direct, high-acuity patient care, with a focus on wound care

• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.

• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.

Requirements

• Active Michigan RN license

• Reliable transportation, valid driver\'s license, auto insurance

• Home health / OASIS experience strongly preferred
