Job summary

Adaptive Home Health, LLC is seeking an On Call Registered Nurse (RN) - Case Manager to deliver high-quality in-home care in Detroit. This field-based role includes after-hours on-call shifts, part-time as needed, with compensation structured per point and mileage reimbursement for travel to patient sites.

Responsibilities include comprehensive in-home assessments, high-acuity patient care with wound care focus, and coordinating the plan of care with physicians while guiding LPNs and HHAs.

Qualifications

• Active Michigan RN license.
• Home health experience preferred.
• Reliable transportation and valid driver's license.
• OASIS documentation experience preferred.

Responsibilities

• Deliver in-home nursing care with emphasis on wound care and accurate OASIS/recert documentation.
• Own patient outcomes by monitoring and improving visit quality and reducing avoidable hospitalizations.
• Collaborate with physicians to build and manage the plan of care; supervise LPNs and HHAs on your cases.
• Represent Adaptive to patients, families, and referral sources to build trust and referrals.

• Additional compensation for any services provided after hours.
• $0.67 cents per mile for mileage reimbursement
• Schedule: On Call After Hours/ Part Time (as needed)
• Location: Field-based
• Comp: $64.10 per point

The mission of this role

Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on.

Objectives

• Carry a high-quality caseload: at target visit quality, with OASIS accuracy that holds up to review.
• Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality.
• Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent.
• Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows.

What you'll own

• Comprehensive in-home assessments and skilled documentation (OASIS, SOC/recert/ROC, discharge).
• Direct, high-acuity patient care, with a focus on wound care
• Building and managing the plan of care with physicians; supervising LPNs and HHAs on your cases.
• Being the face of Adaptive to patients, families, and the referral sources who feed your pod.
